Sokil Model Release Form

Dear Plastuny and Parents,

My name is Andrea Wenglowskyj, and I am a Ukrainian-American artist and former Novyi Sokil camper from 1986-1996. I have such wonderful memories of this place. My art work explores the Ukrainian-American diaspora and I will continue documenting the happenings at Sokil this summer of 2024. I am devoted to helping preserve and share this amazing opportunity and tradition that we all share.

It would be wonderful to use all campers as subjects in my photographs as I observe and document tabir this summer. If you agree, please fill out the information below. This will authorize me to use these photographs in artistic endeavors, which could include, but not limited to: exhibitions, print (book, media), and online features. Please do not hesitate to contact me with any questions.
